Lifetime Care Services’ mission is to help combat healthcare inequities in America’s most underserved communities. We partner directly with clinics that serve Medicare patients with two or more Chronic Care Conditions and/or Behavioral Health disorders.
Our services augment patient care and provide additional revenue streams to clinics by maximizing Medicare-funded reimbursements.
Healthcare Disparaties
Among Medicare beneficiaries, people of color are more likely to report being in relatively poor health, and have higher prevalence rates of some chronic conditions, such as hypertension and diabetes than White beneficiaries.
The World Health Organization defines these disparities as the “differences in health which are not only unnecessary and avoidable but, in addition, are considered unfair and unjust”.
Our goal is to help close this gap by providing patients qualified telehealth professionals 24/7 who maintain continuous contact in between visits to the clinic.
Spanish-dominant Americans prefer Hispanic health care providers
About eight-in-ten (81%) Hispanic adults who are Spanish language dominant prefer to see a Spanish-speaking health care provider, underscoring the potential for communication issues in receiving medical care.
